Parents/Guardians,

Please use the following form to register for After-School Care at Liberty Primary School! Your child's spot in the program is NOT reserved until the supply fee is paid either online via MySchoolBucks, or cash or check to the front office, AND you have received confirmation by one of the directors.

HOURS:

Available until 6:00pm on all school days.

After-care will NOT be available when school is operating on half-day schedules, closed, or dismissed early.

A late fee of $1.00 minute will charged if pickup is after 6:00pm.

FEES:

Supply Fee -

$60 supply fee per child, due prior to attendance.

Tuition Fee Schedule -
$60 per week/one child - Full Week Tuition

$110 per week/two children

$165 per week/three children

** If part time care is needed please contact one of our directors for assistance.

$15.00 late fee - Will be charged on all accounts that are not paid by Wednesday prior to the week of attendance.

PICK-UP POLICY:

Pick-up will be at the front entrance of the school using the CODE word that was provided on your registration. Whoever picks up your child WILL need to provide that code word for pickup.
